Pathways to Independence specializes in supporting individuals
with an Acquired Brain Injury. Our programs and services are
tailored to accommodate individual needs and provide a continuum
of care. We recognize that people’s needs change over time and
our services and support adapt on a continuous basis to meet
a person’s changing requirements.
Fully-accredited by the Canadian Council of Health Services
and a preferred service provider of the Ministry of Health and
Long Term Care, Pathways to Independence is regularly assessed
and evaluated against exacting standards of care. These standards
ensure that we can provide the highest quality of residential
services for the people we support, their families and third
party funding agencies.
Our residential options offered throughout south- eastern Ontario
in both rural and urban settings include:
• 24 hour supported homes
• Supported Independent living in apartments
• Family Homes
• Respite services
We use comprehensive planning processes to support a person’s
rehabilitation and reintegration to the community following a
brain injury. In addition to assisting with activities of daily
living, our staff access broad social networks and community
partners to develop and support the implementation of a person’s
individualized plan.
Depending upon the nature of the brain injury, these supports
may include:
• Physiotherapy
• Occupational Therapy
• Speech Therapy
• Psychological & Psychiatric Services
• Nursing
• Dietitian
• Adult Education, Vocational Training or Upgrading
• Family support
Pathways to Independence’s Club ABI provides a relaxed setting
with opportunities for people with Acquired Brain Injury to develop
new skills, build social networks and have fun.
We understand the unique needs and requirements of the ABI community.
We offer innovative programs and services that rebuild skills,
abilities and relationships further enabling the people we support
to achieve their goals.
