Discover Pathways’ Programs And Services

Driven by client centered principles, Pathways employees work closely with the people we support to help them achieve their goals, develop skills and foster independence in the communities where we live, work and play.

Pathways to Independence provides case management and assisted community living services and supports for adults with an acquired brain injury, a dual diagnosis or a developmental disability. Case management assists a person to identify, obtain, and to coordinate multiple services. This includes both direct and indirect service. A direct service could be assistance with personal finances. An indirect service could be helping the person to access a medical specialist.
