Developmental and Dual Diagnosis Services

Accessing services for adults with developmental and dual diagnosis disabilities is managed by the Developmental Service Ontario (DSO). There are nine regional offices that will assist adults with developmental disabilities, their family members and/or caregivers to understand how to access and pay for services and supports.

Acquired Brain Injury (ABI)

An Application for Service form must be completed if you wish to apply for services. The form can be completed by a survivor, family member or professional involved. Each application will be reviewed on an individual basis and if approved for services, any applicable fees will be determined at that time.

Please complete the attached application and mail to the address on the form. You must include the signed consent for your application to be accepted. If you are unable to download the application, one can be mailed to you upon request.
