Privacy Statement
Pathways to Independence is committed to protecting the privacy, dignity, and confidentiality of the individuals we serve and those who interact with our organization. We are dedicated to ensuring personal, identifying and sensitive information is protected, and that our collection, use, retention and disclosure of such information is carried out in accordance with the established principles and legislation.
Legal Authority
The collection, use, and disclosure of personal health information is governed by the Information and Privacy Commissioner of Ontario and the Personal Health Information Protection Act (PHIPA), and other applicable Ontario and federal legislation.
As an agency funded by the Ministry of Children, Community and Social Services (MCCSS) and the Ministry of Health (MOH), we are committed to meeting all legislated privacy obligations.
Our Commitment
We collect, use, and disclose personal information and personal health information only for purposes that are lawful, relevant, and necessary to provide quality services and supports. The collection, use, and disclosure of personal information is limited to what is necessary for the provision of services and is conducted in accordance with applicable legislation.
Your Rights
You have the right to:
- Know why your information is being collected and how it will be used
- Access personal information we hold about you (note that there may be a cost associated with requesting access to your information in accordance with IPC Orders)
- Request corrections to information that is inaccurate or incomplete
- Withdraw consent where applicable and be informed of any consequences of doing so
- File a complaint if you believe your privacy rights have not been respected
Our Obligations
Pathways to Independence is funded by the Ministry of Children, Community and Social Services (MCCSS) and the Ministry of Health (MOH). We are committed to fulfilling our obligations under the Personal Health Information Protection Act (PHIPA) and all other applicable provincial and federal privacy legislations.
All staff are required to maintain the confidentiality of personal and personal health information and are trained in their privacy obligations.
